While booking, you will be asked to enter your credit card information to secure your appointment.  Booking through my website is required before 5:00pm on the evening prior to our visit to reserve your appointment.
*If you are approved for insurance coverage, a 100% discount will be applied. 
*If you are on WIC (and not approved for insurance coverage) please provide a copy of your WIC card and note this in the "appointment notes" section while booking and a discount will be applied to your charge, this discount is honored for office and virtual appointments only.
*Twins/Multiples will have a $100 surcharge added to initial and follow-up visits (private pay).
    

Home visit availability may be limited.  If you are needing a home visit, please message me prior to booking.  Home visits are offered within a 15 minute radius of Hammondsport, NY and within a 15 minute radius of Penn Yan, NY.  Home visits will incur a $50 travel fee.

Visits beyond this radius may be available on a case by case basis and will incur an additional travel fee.
